October is Breast Cancer Awareness Month and National Physical Therapy Month, so what’s better than combining the two for an article? A patient contacted the office this week post-mastectomy, and she said she “was having a hard time finding a physical therapy facility that performs post-mastectomy physical therapy.” This article is to bring awareness and explain the benefits that physical therapy can offer, at Prime Orthopedic Rehabilitation, after a mastectomy and lumpectomy.

A mastectomy is a significant surgical procedure, commonly performed to treat or prevent breast cancer. The surgery involves the removal of one or both breasts, either partially or entirely, which can lead to physical, emotional and psychological challenges. Physical therapy (PT), helps patients regain fascia and joint mobility, strength and function, while preventing or addressing common post-surgical complications.

 

Why Is Physical Therapy Important After a Mastectomy?

Post-mastectomy, patients often face several physical limitations and discomforts. They often experience limited range of motion of shoulder and arm mobility on the side of the surgery, which can be restricted due to scar tissue formation and discomfort, and can sometimes lead to limitations in the joint capsule, further restricting movement. Pain and tightness can occur due to scar tissue, muscle tension and nerve damage, and can sometimes cause numbness. Patients may experience weakness and reduced strength in the chest, shoulder and arm muscles, which can lead to decreased function, poor posture due to muscle imbalances in the chest muscle and abnormal compensatory movements, which can then affect you later in life. These limitations can also compound the emotional impact of losing a breast. In addition, lymphedema is a common complication where lymph fluid buildup causes swelling in the arm, hand or chest, due to lymph node removal. Physical therapy addresses these challenges and offers a structured way to promote healing, alleviate pain, and improve overall quality of life.

 

Benefits of Physical Therapy After Mastectomy

One of the primary goals of PT is to help patients regain full mobility in the shoulder and arm, which can become restricted due to scar tissue and stiffness. PT exercises gradually stretch the muscles and joints, allowing patients to move freely again. Mastectomy can lead to chronic pain due to nerve damage or muscle tightness. Physical therapists may use soft tissue mobilization, stretches and modalities to reduce pain and promote healing. Post-mastectomy, the muscles around the shoulder, chest and upper back may weaken due to surgery and inactivity. Breathing exercises enhance lung capacity and eases chest tightness caused by surgical trauma. Physical therapy includes strengthening exercises that target these areas, helping patients rebuild their strength and endurance. Poor posture can result from the physical and emotional changes post-surgery, leading to muscle imbalances and discomfort. PT helps correct postural issues by strengthening the core and upper body muscles, improving alignment, and reducing strain on the shoulder, spine and neck. For patients who have undergone lymph node removal, the risk of lymphedema is higher. Physical therapists use specific massage techniques, known as manual lymphatic drainage, along with myofascial release techniques, and exercises that stimulate lymph flow to help manage or prevent swelling.

Physical therapy also plays a critical role in addressing the emotional toll that a mastectomy can take. Lastly, and possibly most importantly, the return to physical activity and regaining functional independence boosts confidence and contributes to a more positive body image, aiding emotional recovery.

 

When to Start Physical Therapy

The timing of starting physical therapy varies based on the surgeon’s recommendation, but it is typically advised to begin within a few days to a few weeks after surgery, once incisions have healed sufficiently. Early intervention can prevent complications and expedite recovery.

Physical therapy is a vital component of post-mastectomy care, promoting healing, reducing pain and restoring function. It empowers patients by providing tools to manage the physical and emotional challenges associated with breast surgery. It’s essential to work closely with a healthcare team to ensure that therapy is safe and effective. A tailored program should be designed to meet the individual’s needs and recovery goals.
